Academic Reinstatement Appeal

Please note, any student placed on academic suspension for the first time, must sit out for at least a 16-week semester. Any student placed on academic suspension for a second time, must sit out for at least two consecutive 16-week semesters. Any student placed on academic suspension for a third time, is ineligible for reinstatement.

All students must complete this form in order for their academic reinstatement appeal to be processed.
