Transcripts/Records

At Hopkinsville Community College, our Admissions & Records Office offers a combined service to meet all of your student admissions and records needs.

How do I request a transcript be mailed somewhere?

If you would like to request a transcript be sent to an institution, organization, or another location, you'll need to:

  • Complete our transcript request form. PDF icon
  • Send us your own form which includes your name and/or previous names, at least the last four numbers of your Social Security Number, your date of birth, and a way to contact you for any missing information.
  • Be prepared to provide payment.

There is a small fee to process your request and is required each time you request a transcript be sent, those fees are:

  • Mailed: $5.00
  • On Demand: $7.00
  • Faxed: $10.00

In many cases a transcript must be mailed for it to be considered official. Before choosing an on demand or faxed transcript, check with the party who will receive the transcript to be sure they will accept a hand carried or faxed transcript. All official transcripts are processed at the Hopkinsville campus; official transcripts cannot be paid for or printed at the Fort Campbell campus. Please allow 3-5 business days for processing your transcript request.

How do I apply for my credential?

If you are ready to complete your degree, diploma, or certificate, you'll need to:

  • Fill out the application for credential form.
  • Meet with your assigned advisor to make sure you have completed all the courses and requirements needed.
  • Return the application for credential form to the Admissions & Records Office.

Credentials are awarded three times a year at the end of each spring, fall, and summer semesters. You can view the application deadlines for credentials on the academic calendar for each semester. Make sure you have your forms completed and turned in before the appropriate deadline.

Once you have applied for your credential, you will receive an email notification if there is a problem with your application. Otherwise, your credential will be listed on your academic transcript and will be mailed to you after the semester is completed.
